Coordinated Entry System provides comprehensive services designed to address the root cause of poverty and homelessness within SPA 1 of Los Angeles County. Participants may be mentally ill, have intellectual developmental disabilities, substance abuse issues, and chronic medical/physical disabilities, exiting institutions and are homeless.

Under the leadership of the Director of Regional Coordination, Regional Coordinator works with internal and external departments to implement and execute all facets and objectives of the Families Coordinated Entry System. As the designated lead agency in SPA 1, this position will engage the community; coordinating, cultivating, and enhancing relationships with special populations (i.e.-homeless families/children, faith-based organizations, schools and non-traditional partners) toward the common goal of housing the homeless population in the region.  The Regional Coordinator oversees the infrastructure through which the Coordinated Entry System is implemented in SPA 1, with emphasis on access, assessment, prioritization, and matching. 

Essential Job Functions:
  • Coordinate day to day operations of the Families Regional Coordination systems
  • Supervise direct program staff to achieve system objectives by providing supervision and expertise
  • In conjunction with Supervisor; provide regular and on-going feedback to direct staff including training, opportunities for growth, verbal and written improvement plans as necessary and regular evaluations
  • Problem solve with direct staff on any issues or challenges that might inhibit them from achieving system goals
  • Responsible for reviewing staff time sheets and submitting for payroll
  • Responsible for interviewing and onboarding of new staff; Communicate performance expectations and outcomes
  • Responsible for addressing, counseling and resolving staff performance issues and preparing annual staff evaluations
  • Conduct and attend regular supervision meetings to engage staff in critical reflection and aid in professional growth
  • Train, coach, mentor and manage direct staff on program requirements, tools, and methods to meet those requirements set in place by our funders
  • In collaboration with Supervisor, develop and implement policies, procedures, and systems necessary to maintain high-quality programs and services
  • Communicate progress towards achieving program goals and outcomes to Supervisor
  • Oversee, Track and Report on United Way Regional Flex Funds Contract
  • Work with CES system funders, Chief Executive Officer, Program Directors and Finance departments, to coordinate outcomes, budget reporting, monitoring and file review.
  • Provide oversite for all functions of Families Regional system, including contracts and outcomes
  • Work with regional partners to track housing placements and various data points within the region
  • Work with regional community partners to implement and ensure accurate data entry practices.
  • Design, develop, implement, produce and present regular reports of regional data to share information internally and in appropriate forums throughout SPA 1
  • Ensure technical support is readily available to staff, subcontractors and partners, including linkages to trainings and literature on best practices
  • Identify and develop strategies to reach outlying underserved areas of the SPA
  • Serve as the primary representative for the Families Regional Coordination System in SPA 1 to support the expansion of system partners and facilitate the ongoing utilization of the system
  • Provide guidance to staff, subcontractors and partners around policies, procedures and regulations for regional coordination and matching
  • Coordinate Families Regional Coordination activities in SPA 1, implementing the system access points and ensuring that the access points are operating effectively, cooperatively and in a streamlined manner.
  • Work with other Regional Coordinators and leaders to identify programmatic challenges and gaps in services. Strategize on program innovations. Highlight and disseminate best practices.
  • Manage the relationships with CES community partners and actively participate in regional collaborative to ensure services are available to all homeless populations.
  • Identify and incorporate new resources into CES for the SPA
  • Research best practice models for Coordinated Entry Systems and related processes and practices and developing recommendations on how to implement them
  • Develop and strengthen relationships with community groups/organizations working against Homelessness in the City, County and State
  • Conduct at-least quarterly meeting with community providers to ensure a consistent feedback loop to LAHSA and CES regarding the implementation of the Families Regional Coordination Entry in the region. This includes evaluating the progress of the Families system, identifying gaps, and increasing linkages to key support services resources.
  • Coordinate with community partners and stakeholders to increase the knowledge, availability and accessibility of supportive services and housing opportunities for people experiencing homelessness and housing instability in the region.
  • Represent Valley Oasis and SPA 1 in the community educating stakeholders on the Coordinated Entry System
  • Coordination and facilitation of Families Regional Care Coordination and Case Conference meetings
  • Attend funder meetings/trainings and community events as required by supervisor to maintain network of resources and services
  • Participate in case conferencing meetings to provide input on resources available with the staff to divert families from entering CES system and advocate for vulnerable participants who would benefit from CES program
  • Participate, attend Care Coordination meetings to network with community partners, and acquire resources that may be useful for families seeking services
  • Expand the network of Service Providers in SPA 1 who attend and participate in the bi-weekly Care Coordination for the Families populations
  • Participate in staff meetings, individual and group supervision, case conferences and training as required
  • Ensure ongoing and consistent team, subcontractor and partner meetings and case conferences to provide program guidance, encourage collaboration, and share best practices.
  • Ensure timely and accurate submission of required funder reports.
  • Adhere to agency policies and procedures including confidentiality, child/elder abuse reporting, and code of ethics
  • Represent the Agency at various meetings held in the community; be an active participant in the community; act as a link to the community and the Agency
  • Additional tasks, projects and or responsibilities as assigned by Supervisor that are not outlined but which are an important part of the agency’s goals and/or program outcomes
 Knowledge, Skills & Abilities:
  • Clear understanding of the concepts of Housing First, Harm Reduction, Trauma Informed Care, Motivational Interviewing, and the Coordinated Entry System
  • Knowledge and understanding of policy, systems, and local programs that impact homeless persons and related public funding sources, regulations, requirements, and procedures
  • Knowledge of best practice models for assisting homeless people with varying degrees of needs/conditions and have a strong understanding of contractual requirements/compliance and applicable performance measures
  • Knowledge of issues facing homeless individuals (e.g. chronic health, substance abuse, mental health, domestic abuse and resources for undocumented individuals)
  • Knowledge of community social service agencies and resources
  • Work effectively under conditions of limited supervision, high stress and rapidly changing situations and circumstances
  • Ability to handle common problems without supervisor intervention while knowing when supervisor participation is warranted
  • Must be able to exercise initiative, independent good judgment, flexibility, discretion, and solid decision making
  • Ability to respond quickly and effectively to individuals who may be ill, using substances, lack of personal care, in crisis situations and/or potentially hostile situations
  • Ability to work with a diverse population and provide culturally competent advocacy
  • Actively promote positive, nurturing interactions between participants
  • Ability to deescalate aggressive/agitated individuals
  • Able to take appropriate steps to deal with emergency situations with the goal of maintaining the safety of all participants
  • Must be able to successfully manage multiple priorities, meet deadlines, adjust to changing; circumstances, and demonstrate confidence working with diverse stakeholders
  • Demonstrate the ability to utilize strong judgement and interpersonal skills to manage sensitive client interactions including with those who may be in distress with acute illness, substance use issues, co-morbidity of behavioral and physical health issues in addition to homelessness
  • Project a professional demeanor and a high level of professionalism when dealing with sensitive matters
  • Outstanding communication skills, both written and oral; must communicate professionally and positively with participants, co-workers, management, community partners, employers, landlords, and other service providers
  • Promote cooperation and commitment within a team to achieve goals and objective; collaborate with team members, sharing ideas and differences openly; be receptive to new ideas and adapt to change as necessary
  • Strict confidentiality must be observed as it relates to client information, agency business, program decisions and all other personal information
  • Considers impact of personal and professional choices. Consistently make decisions in keeping with organizational values and Supervisor’s direction
  • Excellent organizational skills and the ability to follow through from beginning to end on tasks
  • Proficient with office communication equipment and machines (telephone, copier, calculator, fax machine, personal computer, laptop, computer printer and scanner)
  • Proficient in use of laptop/desktop computer, Microsoft Windows and Office software programs (Word, Excel, and PowerPoint) and Google drive
  • Strong time management skills and the ability to multi-task and prioritize in a fast-paced environment, while exercising sound judgment
  • Able to work a flexible schedule; including nights, days, weekends and holidays when needed
  • Able to respond to emergencies some evenings and weekends as necessary
  • Use of a personal vehicle may be required to travel in and around Antelope Valley and/or Los Angeles County and periodically transport participants
  • Maintain professional boundaries and work collaboratively with participants, volunteers and staff
  • Accountability – be on time for scheduled shifts
  • Be able to read and write English
  • Bilingual Preferred but not required
Qualifications & Experience:
  • Bachelor’s Degree in Social/Human Services or related field preferred
  • Minimum three years’ relevant work experience in the provision or oversite of crisis-bridge housing, rapid re-housing and/or permanent housing programs, preferred
  • Minimum two year experience in supervision or leadership role for interim housing, rapid re-housing or permanent housing programs
  • Minimum one year experience working with and managing grants from City, County, State and/or Federal funding agencies
  • Strong knowledge of the social services field and/or successful management and implementation of homeless programs in the non-profit, government or private sector
  • Experience with Homeless Management Information System (HMIS)
  • Education may be substituted by years of experience

  • Employee will be required to complete a TB test, fingerprinting, drug screen (employment pending on drug screen, background and Motor Vehicle Report review), 72-hour Domestic Violence/Sexual Assault training and CPR/First Aid certification

Schedule
Monday – Friday 8am – 5pm (may be required to work other than normal duty hours, to include evenings, weekends, holidays and/or on call status) On – call when needed
 
Salary Range
$63,000 - $70,000 annually (Exempt) **based on experience
